SSVF Housing Based Case Manager — Manistee/Cadillac area

The Case Manager will collaborate with the Northwest Michigan Coalition to End Homelessness (NMCEH)

and the Michigan Balance of State Continuum of Care (BOSCOC) to implement the Housing and Homeless

Services programs of the Northwest Michigan Community Action Agency (NMCAA). This position involves

direct engagement with the homeless population through community shelters and street outreach, assisting

clients in securing safe, permanent housing, and connecting them with additional community resources for

support. The Case Manager will also be responsible for maintaining client information, notes, and housing

plans, as well as completing reports for various programs and grants while advocating for effective solutions

to end homelessness.

This position will primarily serve clients in Manistee, Mason, and Oceana counties, with

additional support (as needed) for clients in Benzie and Grand Traverse Counties.
The ideal candidate will reside in or near Manistee, with office hours (as applicable) held in Cadillac.
