School Success Liaison - Long Lake Elementary School

Job Summary:
The School Success Liaison serves as a connection between families, schools, and community agencies, ensuring support for students and families to foster academic success. This role involves developing action plans, facilitating communication, coordinating services, and addressing attendance and truancy concerns for families in the program.

Duties/Responsibilities:

Develop and implement an action plan with the school and family to support student success, including weekly in-school contacts with students and bi-weekly contact and/or visits with parents. 

Continuously update direct supervisor on concerns, issues, and caseload status.

Enhance communication, networking, and collaboration between parents, schools, and community agencies.

Provide support services and follow-up activities in both home and school settings.

Upon receiving a referral, convene a meeting with all involved parties to collaboratively create an action plan and schedule the first parent visit.

Maintain contact with and schedule and conduct visits with parents at least bi-weekly to gather information, set goals, and assist with problem-solving.

Meet weekly with teachers to assess student progress and adjust action plans as needed.

Meet weekly with referred students in the school setting.

Represent the agency, school district, and program professionally at all times.
Seek consultation and make referrals to community agencies as necessary.
Coordinate services for families involved with multiple agencies.
Maintain accurate records and documentation for caseload and support tasks, including EmpowOR data entry as required by supervisor.

Attend staff meetings and training sessions as required by supervisor.

Participate in inter-agency groups and committees when possible.

Assist schools with addressing attendance and truancy concerns as determined by needs of the school; may act as the assigned truancy liaison and participate in truancy meetings, preliminary inquiries, and court hearings as needed.

Education and Experience:

B.A. or B.S. in Social Work, Counseling, Sociology, Psychology, or relevant field preferred. 1-2 years of experience with school-age children preferred.
Experience working with low-income/at-risk families.
