Street Outreach Housing Navigator

Job Summary:

The Diversion & Outreach Case Manager will work in partnership with the Northwest Michigan Coalition to End Homelessness as well as the Michigan Balance of State Continuum of Care to administer Homeless Prevention programming offered through NMCAA. The Case Manager will act as a leader in the community advocating for an end to homelessness. The Case Manager will work in partnership with local community shelters, and with community partners to administer Diversion and Outreach services.

Duties/Responsibilities:

  • Utilize a positive person-centered approach, emphasizing an individual's strengths and assets in their housing case plan.
  • Develop a person-centered case plan to promote self-sufficiency, independence, and financial security. (Life Skills, Education, Employment, Community Supports, Mental and Physical Health and Wellness.)
  • Demonstrate a proficiency in problem-solving skills and utilize those skills to effectively work with youth.
  • Utilize active listening skills to build a relationship with program participants built on trust.
  • Demonstrate ability to locate and connect individuals with resources in a timely manner.
  • Develop and maintain a robust list of community partners.
  • Coordinate and collaborate with other Northwest Coalition members to End Homelessness for individuals and community partners with the best outcomes for those served.
  • Work within the Rapid Exit Program at shelter in constant coordination with other street outreach staff to avoid entry into shelter or have an individual rapidly exit shelter within the initial 15 days of shelter.
  • Assist individuals to identify options and solutions outside the homeless system to solve their housing crisis.
  • Help individuals locate suitable housing, mediate with landlords and provide client centered assistance in maintaining housing.
  • Participate in case conferencing weekly with team members through a solution-focused positive individuals-centered lens.
  • Attend wrap-around meetings and coalition meetings with community partners.
  • Work with and enroll eligible individuals and families to the MSHDA Housing Choice Voucher with Homeless Preference waiting list.
  • Complete a Service Prioritization Decision Assistance Tool (SPDAT or F-SPDAT) for all enrolled clients to use as a tool in developing a housing plan and retaining permanent housing.
  • Complete and maintain the Homeless Management Information System (HMIS) client profile. All client files are to be maintained using a paperless system and electronic forms as provided. Information and case notes to be completed within 24 hours of the event. Complete entry/exit and SPDAT live as they meet and work with their clients.
  • Perform other duties as requested.
