What We're Looking For

• Bachelor's degree in social work, psychology, human services, or a related field preferred.
• Two years of relevant experience preferred.
• Experience working with Veterans, homelessness, housing instability, healthcare navigation, or other complex social service needs is a plus.
• Strong communication, organization, documentation, and problem-solving skills.
• Ability to build trusting relationships while maintaining appropriate professional boundaries.
• Self-motivated and able to work independently while collaborating with Veterans, VA staff, landlords, service providers, and community partners.
• Comfortable navigating difficult conversations, complex situations, and changing priorities.
• Commitment to respectful, person-centered, and non-judgmental service.
• Valid driver's license, vehicle insurance, and reliable transportation required.
• Ability to travel throughout the service area and meet Veterans in community settings year-round.

• Duties/Responsibilities:
  
• Develop individualized housing plans, including crisis planning, budgeting, and goal-setting.
  
• Assist with housing searches, rental applications, and placement on MSHDA and VASH voucher
  waitlists.
  
• Provide direct financial assistance through SSVF and related programs, ensuring compliance with
  grant guidelines.
  
• Maintain accurate documentation in HMIS and complete assessments such as the SPDAT.
  
• Collaborate with landlords, shelters, and service providers to ensure coordinated support.
  
• Participate in outreach, community meetings, and case conferencing to identify and serve at-risk Veterans.
• Conduct assessments with Veterans and care teams to identify barriers to housing and healthcare.
  Link Veterans to supportive services, including healthcare, mental health, financial aid, and
  transportation.
  Uphold confidentiality, HIPAA, and ethical standards in all aspects of service delivery.
  Attend training and meetings as required to stay updated on best practices and program guidelines.
  Other duties and responsibilities as required.

Position Details & Benefits

Full-time | 40 hours per week | 52 weeks per year
$20.97 per hour
Work setting: Combination of remote, in-office, and community-based work depending on program and Veteran needs.

This position provides coverage throughout Northeast counties in Michigan's Lower Peninsula, with regular travel required to meet with Veterans and community partners.
