Housing Based Case Manager - Traverse City

The Case Manager will collaborate with the Northwest Michigan Coalition to End Homelessness (NMCEH)

and the Michigan Balance of State Continuum of Care (BOSCOC) to implement the Housing and

Homeless Services programs of the Northwest Michigan Community Action Agency (NMCAA). This

position involves direct engagement with the homeless population through community shelters and

street outreach, assisting clients in securing safe, permanent housing, and connecting them with

additional community resources for support. The Case Manager will also be responsible for maintaining

client information, notes, and housing plans, as well as completing reports for various programs and

grants while advocating for effective solutions to end homelessness.

BA/BS in psychology, social work, human services, or a related field preferred.

A minimum of 2 years of relevant experience preferred.

30 hours per week to start, but could be up to 40 as caseloads increase. 52 weeks per year. $19.86 per hour.
